Nippon Gohsei is an upper tier COMAH site, based at a facility where shared services, including emergency response, are provided by a third party. They wanted to understand the current position regarding safety critical communications on site and I was therefore asked to conduct an audit of their systems, specifically: shift handover, permit to work and emergency response.
- I developed an audit question set based on the HSE Human Factors Delivery Guide topic 3, COMAH Critical Communications and other relevant guidance.
- I then spent time on site with Nippon Gohsei and the third party provider, covering each section of the audit, reviewing the processes and supporting documentation, and interviewing relevant personnel, including managers, operators, shift supervisors and day support personnel and the third party provider.
- I provided a detailed report including an overall assessment rating on the state of the systems and recommendations for improvement.
Nippon Gohsei had a detailed report which gave them a rating for their current status, identified any gaps in their systems and provided a set of recommendations to address the issues. They now have the opportunity to develop their plans before the scheduled HSE inspection on this topic.
